Wellness Matters recognizes that many people with commercial insurance have high deductibles. Unfortunately, our contracts with these companies prohibit discounting patient’s out-of-pocket expenses. If you anticipate difficulty in paying your bill due to a high deductible, Wellness Matters has several payment plan options available.
Debit/Credit Card Information Required
Wellness Matters LLC accepts cash and all major debit/credit cards for co-pays and services. Our HIPPA-compliant health record system securely stores card information for convenient electronic billing. Because both public and private insurance plans include a client co-pay or co-insurance responsibility, all clients are required to enter debit/credit card information for electronic billing purposes.
Self-Pay Clients
Since there are significant administrative savings to Wellness Matters LLC if we don’t have to bill your insurance company, we pass along the cost savings to you if you have insurance but choose not to use it. Self-pay clients are charged the same day of service

Uninsured Clients May Be Eligible for Sliding Fee Discount
Uninsured self-pay clients may request a Sliding Fee Discount application if they believe that their total household income is 200% or less than the Fedeal Poverty Guidelines.
Qualifying for the Sliding Fee Discount involves completing the Sliding Fee Discount application and showing proof of gross income for all immediate family members in your household. Gross income is all income from all sources before taxes. Wellness Matters’ Sliding Fee Discount program applies to uninsured clients whose household income is 200% or less of the Federal Poverty Guideline.
Please contact Wellness Matters staff for a Sliding Fee Discount Application if you believe that you will qualify AND you do not have health insurance.
